Transjugular extrahepatic portosystemic shunt (TEPS) creation in patients with complete occlusion of portal vein: Primary experience in a single medical center

Portal hypertension can be caused by many diseases, including cirrhosis, portal vein thrombosis, portal vein tumor thrombus, with potentially fatal consequences. The treatment of portal hypertension include drug therapy, endoscopic therapy, transjugular intrahepatic portosystemic portacaval shunt (HGPCS), and others. Compared with the other therapies, TIPS is considered as a good way for decreasing the pressure of portal vein and increasing the portal venous inflow. It is an ideal choice for treating variceal bleeding with an excellent hemostatic effect (95%) [1]. (within 72hours) must be considered in high risk patients with cirrhosis in Child-Pugh class C or those in class B who have persistent bleeding at endoscopy [2]. However, TIPS can be challenging in patients who lack available vascular access in the liver, such as those with portal vein occlusion or portal cavernous transformation. Consome cases.
