We report a 48-year-old, right-handed man with Parkinson's disease (PD). Right-hand tremor disappeared after radiofrequency (RF) left ventral intermediate nucleus (Vim) and ventro oralis nucleus thalamotomy; however, left-hand tremor developed after RF thalamotomy. We performed magnetic resonance imaging-guided focused ultrasound (MRgFUS) right Vim thalamotomy. The left-hand medication-refractory tremor disappeared immediately after the procedure without serious adverse events. MRgFUS Vim thalamotomy following contralateral RF thalamotomy could be one of the therapeutic options for bilateral tremor in PD patients.
