Historically, extended resections for T4 non-small cell lung cancers that invade adjacent organs have been associated with significant morbidity and mortality, and poor long-term survival. However, notable improvements in imaging, surgical techniques, and perioperative care during the past several decades have resulted in an increase in survival for highly selected patients. This article provides a critical review of the existing statistical evidence regarding the utility of resections of T4 tumors invading the aorta, pulmonary artery, left atrium, and esophagus.
