Functioning free-muscle transfers are now an important tool in management of patients with brachial plexus injuries. They are indicated for restoration of elbow flexion in patients with delayed presentation (those seen after 6-9 months). Double free-muscle transfers provide the possibility of grasp and release function of the hand with active elbow flexion and extension when combined with nerve transfers or grafts for restoration of shoulder motion, hand sensation, and triceps function.
