Associations between opioid substitution therapy and death following imprisonment cannot be explained simply in terms of fluctuations in opiate tolerance. Epidemiological studies using administrative data can identify possible risk factors. To have clinical impact, however, hypothesis-driven research studies are needed which can explore more effectively the multi-factorial nature of these deaths.
