Electrical incidents provide a good learning opportunity. If an organization fails to capitalize on this opportunity, the incident could recur. An abundance of literature is available on the investigation process and various investigation techniques. There appears to be limited guidance on determining the effectiveness of the application of the investigation process after the investigation is complete. As part of a larger research project, a comprehensive study was undertaken at a steel plant in South Africa to address this gap. The study analysed the organizational policy, completed investigations and surveyed investigators all to a common set of requirements established from literature. This paper focuses exclusively on the investigator survey. The primary goal is to share the top findings from the survey, in tandem with other insights and recommendations on possible solutions. A secondary aim is to discuss the process so that it can be repeated in any heavy industry. This will facilitate a discussion on whether the findings presented in this paper are localized to the case study plant or are global challenges to effective incident investigations.
