END-STAGE LUNG DISEASE may be treated with cadaveric lung transplantation (CLT) in adults and children. However, the number of people awaiting transplantation far exceeds the donor pool; 30% to 50% of patients listed for CLT may die because of increasing wait list times.(1,2) An alternative to CLT is living-related lobar lung transplantation (LRLLT). First developed in children,(3) it has now been extended to adults,(4) and there is interest in expanding LRLLT to increase donor organ availability. These donors and recipients have unique perioperative anesthetic considerations.
