Was ist neu? Epidemiologie und Risikofaktoren Bei Personen mit HIV ist das Risiko fur ein Non-Hodgkin-Lymphom (HIV-NHL) bzw. Hodgkin-Lymphom (HIV-HL) auch unter kombinierter antiretroviraler Therapie um das 10-20- bzw. 5-15-Fache erhoht. Wichtige Risikofaktoren sind der CD4-Zell-Nadir und die kumulative HIV-Viruslast. Prognose Die Prognose wird durch den pathologischen Subtyp und den Internationalen Prognose-Index sowie durch die Zahl der CD4-Zellen bei Lymphom-Diagnose und durch molekulare Marker wie die Expression von MYC oder BCL2 bestimmt. Therapie Standardtherapie des diffus gro ss zelligen B-Zell-Lymphoms sind 6 Zyklen R-CHOP. Patienten mit Burkitt-Lymphom erhalten eine Therapie nach dem B-ALL/NHL-Protokoll der GMALL. Fur das plasmoblastische Lymphom ist keine Standardtherapie etabliert, Therapie der Wahl sind CHOP oder EPOCH plus Bortezomib. Die Prognose von Patienten mit primarem Erguss-Lymphom ist weiterhin ungunstig. In kleinen Fallserien wurden relativ gute Ergebnisse mit DA-EPOCH Bortezomib erzielt. Die Ganzhirnbestrahlung ist bei Patienten mit HIV und primarem ZNS-Lymphom obsolet. Protokolle mit HD-MTX sind gut machbar und effektiv. Hodgkin-Lymphom: Stadien-adaptierte Therapie mit ABVD und/oder BEACOPP basis. Bei Patienten mit rezidivierter oder refraktarer Erkrankung kommt eine Hochdosischemotherapie mit autologer Stammzelltransplantation zum Einsatz. Zudem gibt es zahlreiche Fallberichte zur erfolgreichen Therapie mit Nivolumab oder Pembrolizumab. Rezidivtherapie Die Rezidivrate nach kompletter Remission betragt bei Patienten mit NHL ca. 11%. Standardtherapie ist die autologe Stammzelltransplantation, sofern Patienten dafur geeignet sind. Eine Therapie mit CAR-T-Zellen sollte HIV-Patienten nicht vorenthalten werden. Abstract The risk of malignant lymphomas is markedly increased in HIV-infected persons even in the era of effective combination antiretroviral therapy. Major risk factors are the depth of CD4-cell nadir and HIV viremia. R-CHOP remains treatment of choice for diffuse large B-cell lymphoma. In HIV-Burkitt lymphoma CODOX-M/IVAC proved superior to EPOCH in a large retrospective study. Standard treatment for plasmablastic or primary effusion lymphoma has not yet been defined. Favourable results have been reported with a stage adapted treatment for HIV-Hodgkin lymphoma. Patients with relapsed/refractory HIV-lymphoma should undergo autologous stem cell transplantation if indicated. Successful anti-CD19 CAR T-cell therapy was also reported in HIV-infected patients with refractory B-cell lymphoma.
