Intraoperative mitomycin C in endolymphatic sac surgery for Meniere's disease

Endolymphatic sac (ES) revision surgery revealed Fibrosis, scarring and osteoneogenesis around the sac. These revision findings seem to be causative of the poor long-term results of the ES surgery. In order to solve this problem, we tried to use mitomycin C (MMC) for the ES surgery as used in glaucoma filtering surgery. This surgical procedure was approved in advance by the Ethics Committee for Human Research at our university. So far, 16 patients with Meniere's disease underwent this surgical procedure, and the follow-up periods are between 3 months and 20 months (mean, 10.97 months). Some patients complained of slight vertigo during 1 to 2 postoperative months, however, no patients experienced vertigo later al though 3 patients had slight to moderate dizziness at the time of quick head movement. Five patients out of 16 showed hearing improvement from 10 dB to 35 dB and they maintained almost constantly its level. These patients with hearing improvement showed decrease of tinnitus and 2 patients of them showed disappearance of their tinnitus. So far our results seem to be better than our previous conventional ES surgery. Long-term follow-up is necessary.
