Severe Pseudomonal Keratitis in an Infrequent Daily Disposable Contact Lens Wearer

Purpose: To report an unusual case of contact lens-associated pseudomonal keratitis in a compliant daily disposable contact lens wearer. Methods: A case report is presented of a compliant daily disposable contact lens wearer who developed culture-positive pseudomonal keratitis. Results: A 38-year-old woman who reported compliant and infrequent use of daily disposable contact lenses presented with rapid-onset, severe keratitis consistent with pseudomonal infection. Corneal cultures were positive for Pseudomonas aeruginosa. She had no identifiable risk factors, although notably had a remote history of contact lens-associated keratitis in the fellow eye. Conclusions: Although extremely rare, pseudomonal keratitis should still be considered in the differential diagnosis of severe keratitis in daily disposable contact lens wearers. Given the history of a previous contact lens-related bacterial keratitis in this compliant patient, it is possible that host susceptibility factors played a role in the case of pseudomonal keratitis described here.
