Management of adults with congenital heart disease is complex. Given current surgical procedures in the next decade 1 in 150 adult will have some form of congenital heart disease. Even asymptomatic patients after surgery need a close follow-up. Arrhythmias in congenital heart disease are a major issue as they are the leading cause of death particularly sudden death independently of surgical treatment. Most patients will experience atrial arrhythmias. The critical problem is surgical scarinduced arrhythmias that may lead to syncope or death in up to 8 % of patients after surgery for tetralogy of Fallot or Fontan derivation. It is of the highest importance to serially evaluate the risk for sudden death in such patients. (C) 2009 Elsevier Masson SAS. All rights reserved.
