EVALUATION OF HEART RATE VARIABILITY AND ARRHYTHMIA DURING THE HEMATOPOIETIC CELL TRANSPLANTATION IN PATIENTS WITH HEMATOLOGIC MALIGNANCY

摘要
Objectives: Hematopoietic cell transplation (HCT) is a very difficult process for the whole body and especially the heart. We hypothesized HCt might cause arrhythmia and heart rate variablity (HRV). In our study, we evaluate to the effect of only HCT on HRV and arrhymia in patients with hematologic malignancy. Methods: This prospective observational study includes a total of 41 hematologie malignancy patients. All patients were monitored 24 hours with electrocardioagram (ECG) Holder just before and after the HCT. The mean RR interval, the standard deviations of all normal-to-normal RR intervals (SDNN, the standard deviations of 5-minute mean RR intervals (SDANN), the mean of the standard deviation of all normal-to-normal RR intervals for all 5-minute segments (SDnn index), the percentage of successive normal RR intervals exceeding 50 ms (pNN50), the square root of the mean of the squares of the differences between successive normal-to-normal RR intervals (RMSSD), and arrhythmias were calculated from Holder records. Results: the 24-hour analysis of HRV related that the mean HR, mean RR interval, and SDANN are significantly significant. Superaventricular tachycardia episodes, extra vanticular beats, and extra atrial beats were seen frequantly after the HCT. Seven patients have died after HCT. These patients had significantly higher heart rate and atrial fibrillation ratio. Conclusion: Our data suggest that arrhythmias and decreased HRV were observed after the HCT, and they be associated with cardiac mobidity and mortality.
