Pediatric pituitary surgery has evolved to address a wide range of pathologic conditions, and the techniques developed over the past decade have allowed for access to these lesions, enabling surgical results once thought impossible. Concerns regarding the lack of sphenoid pneumatization and risks of CSF leak have been overcome with advances in technology and technique. A complex understanding of the pathologic conditions that occur in this region is important for an appropriate workup and determination of initial surgical intervention. Although controversies still exist, especially with regards to the management of pediatric craniopharyngioma, endoscopic endonasal approaches to pediatric pituitary and sellar lesions have become well-established tools that will continue to undergo further advancement and refinement.
