Pilonidal disease affects young people, usually men, and can result in an acute abscess or chronic draining sinus tracts. It is a condition associated with hair penetration beneath the skin of the midline gluteal cleft. No ideal treatment exists. Numerous surgical treatment options from simple unroofing to sophisticated flaps are discussed.
