摘要
A paradigm shift in thinking about postmastectomy radiotherapy (PMRT) has resulted from level I evidence that PMRT improves overall survival when given in conjunction with systemic therapy in 'high risk' pre and postmenopausal patients. The role of PMRT in women at lower risk of loco-regional failure (LRF) with 1-3 involved nodes and node negative with other risk factors is controversial and the subject of a new international trial (SUPREMO). Technical developments in radiotherapy to improve dose homogeneity with in the target volume and reduce irradiation of the heart may contribute to improving loco-regional control and reducing morbidity.
