Life-threatening complications of scleroderma (SSc) that may require ICU admission include undiagnosed or decompensated pulmonary hypertension, acute aspiration, alveolar hemorrhage, renal crisis, and occasionally symptomatic pericardial or bowel involvement. This article focuses on the diagnosis and management of each these scleroderma complications with an emphasis on the ICU evaluation of pulmonary arterial hypertension (PAH).
