We present a simple and general assessment of a patient with neuromuscular disease, so that assistants may understand the patient's status. A patient is assessed from four perspectives; medical(M), psychological(P) and environmental(E) status, and the ability to perform self care(S). Three degrees; mild(1), moderate(2) and severe(3), are provided for each perspective. Name, age, diagnosis and remarks such as artificial ventilation are accompanied. We applied this assessment. For example, a patient with Duchenne muscular dystrophy will show M1P1S2E1 in the early stage and M3P1S3E1 in the late stage. We concluded that our assessment would inform assistants of the patient's status effectively.
