Limited information is available about the rates and risk correlates of socioemotional/behavioral problems in young children in pediatric primary care settings serving low-income families. Our objective was to determine rates of clinically significant socioemotional/behavior problems in 12- to 48-month-olds from low-income families and identify associations between problems and individual and cumulative demographic and psychosocial risks. In this study, 378 Spanish- and English-speaking mothers attending a pediatric primary care practice serving low-income families were surveyed before well-child visits to assess socioemotional/behavioral problems (Brief Infant-Toddler Social-Emotional Assessment; M.J. Briggs-Gowan & A.S. Carter, ) and psychosocial and demographic risks (e.g., unemployment, low social support) (Parent Risk Questionnaire; D.I. Lowell, A.S. Carter, L. Godoy, B. Paulicin, & M.J. Briggs-Gowan, ). We found that 19.8% of children had clinically significant problems, and 53.2% experienced one or more psychosocial risks. Clinically significant socioemotional/behavioral problems were modestly to strongly associated with individual psychosocial risks, with the strongest associations with parental medical problems, parent depression/anxiety, and extreme parental distress, Adjusted Relative Risk (ARR) = 4.8-6.6, p < .0001. Cumulative demographic and psychosocial risk were uniquely associated with clinically significant problems, particularly among children experiencing three to four psychosocial risks, ARR = 3.0-11.6, p < .05. Psychosocial risks affect the majority of low-income families with young children, with a steep increase in likelihood of clinically significant socioemotional/behavioral problems as risks accumulate, underscoring the need to address both socioemotional/behavioral issues and psychosocial risk in young children. Antecedentes: Es limitada la informacion disponible acerca de correlativos tipos y riesgos de problemas socio-emocionales/de conducta en el caso de ninos pequenos en lugares de cuidado pediatrico primario que sirven a familias de bajos recursos. Objetivo: Determinar los tipos de problemas de conducta y socio-emocionales clinicamente significativos en ninos de 12 a 48 meses de edad provenientes de familias de bajos recursos, e identificar asociaciones entre los problemas y los riesgos sicosociales y demograficos tanto individuales como acumulativos. Metodos: Se sometio una encuesta a 378 madres hablantes de espanol e ingles que asistian a una clinica de cuidado pediatrico primario para familias de bajos recursos. Esto se hizo antes de las visitas de bienestar del nino para evaluar los problemas socio-emocionales/de conducta (Breve Evaluacion Socio-Emocional de Ninos Pequenos e Infantes) y los riesgos demograficos y sicosociales (v.g. desempleo, bajo apoyo social) (Cuestionario de Riesgo del Progenitor). Resultados: 19.8% de los ninos tenian problemas clinicamente significativos y 53.2% experimentaba riesgo sicosocial al nivel 1+. Los problemas socio-emocionales clinicamente significativos/de conducta fueron entre modesta y fuertemente asociados con riesgos sicosociales individuales, siendo las mas fuertes asociaciones con problemas medicos del progenitor, la depresion/ansiedad del progenitor, y la extrema angustia del progenitor (ARR = 4.8 a 6.6, p<.0001). El riesgo sicosocial y demografico acumulativo se asocio de forma unica con los problemas clinicamente significativos, particularmente entre los ninos que experimentaban de 3 a 4 riesgos sicosociales (ARR = 3.0 a 11.6, p<.05). Conclusiones: los riesgos sicosociales afectan a la mayoria de las familias de bajos recursos con ninos pequenos con un abrupto incremento en la posibilidad de problemas socio-emocionales clinicamente significativos/de conducta a medida que los riesgos se acumulan, lo cual subraya la necesidad de prestar atencion tanto a los asuntos socio-emocionales/de conducta como al riesgo sicosocial en ninos pequenos. Contexte: Nous n'avons que des renseignements limites sur les taux et les correlations des problemes socio-emotionnels et des problemes de comportement chez les jeunes enfants dans des contextes de cabinets de pediatrie servant les familles de milieux defavorises. Objectif: Determiner les taux de problemes socio-emotionnels et des problemes de comportement cliniquement important chez les enfants de 12 a 48 mois issus de familles defavorisees et aussi identifier les associations entre les problemes et les risques, la demographie individuelle et cumulative et les risques psychosociaux. Methodes: 378 meres hispaniques et anglophones consultant dans un cabinet de pediatrie servant les des familles de milieux defavorises ont ete evaluees avant les visites de bien-etre de leur enfant afin d'evaluer les problemes socio-emotionnels et les problemes de comportement (Evaluation Breve Socio-Emotionnelle du Jeune Enfant) ainsi que les risques psychosociaux et demographiques (comme par exemple le chomage ou un soutien social insuffisant). Resultats: 19.8% des enfants avaient des problemes importants du point de vue clinique et 53.2% d'entre eux ont fait preuve d'un ou plus risque psychosocial. Les problemes socio-emotionnel et les problemes de comportement important du point de vue clinique etaient modestement et cependant fortement lies aux risques psychosociaux individuels, leurs associations les plus fortes venant avec les problemes medicaux parentaux, la depression et l'anxiete parentale et la detresse parentale extreme (ARR = 4,8 to 6,6, p<,0001). Les risques culmulatifs demographiques et psychosociaux etaient uniquement lies aux problemes importants du point de vue clinqiue, particulierement chez les enfants faisant preuve de 3-4 risques psychosociaux (ARR = 3,0 to 11,6, p<,05). Conclusions: Les risques psychosociaux affectent la majorite des familles de milieux defavorises avec de jeunes enfants avec une forte augmentation de la probabilite de problemes socio-emotionnels etant important d'un point de vue clinique au fur et a mesure que les risques s'accumulent, soulignant le besoin qu'il y a d'aborder les problemes socio-emotionnels et le risque psychosocial chez les jeunes enfants.
