Isolated ruptures of the deep medial collateral ligament of the knee (dMCL) are mostly described in a chronic state or in combination with lesions of the superficial medial collateral ligament (sMCL). The case of a 15-year-old male patient who presented to the emergency department after an external rotatory trauma to the knee is reported. Imaging of the knee showed an isolated dMCL rupture. Primary treatment comprised rest, elevation, ointment dressings, oral intake of NSAIDs (nonsteroidal anti-inflammatory drugs) and a knee brace. Five weeks after the injury, the patient presented without any complaints during clinical examination. Based on the current literature, a treatment algorithm for patients with an acute or chronic injury to the dMCL is proposed.
