摘要：
A 50-year-old woman with a history of ventriculopleural shunt placement, presented with nausea, vomiting, fever, respiratory distress and altered mental status. Computerised tomography scan of head revealed presence of air in the ventricles. Source of air was most probably an episode of resolved spontaneous pneumothorax, as no pneumothorax was present in the admission chest X-ray. Shunt revision was planned emergently, and broad-spectrum antibiotics were started. The patient recovered and was discharged in her usual state of health.
