Two case reports reflecting the necessity of surgical treatment for patients with a postacute pseudocyst and a pseudocyst in chronic pancreatitis are presented. Besides other complications, in both cases complete portal vein obstruction causing portal hypertension could be relieved. The treatment protocols exemplify pseudocysts to be only an epiphenomenon of the underlying pancreatic disease. Conclusively, any treatment protocol should aim not only at the pseudocysts but predominantly at duct rupture and all concomitant complications of pancreatitis.
