Hepatic retransplant is the sole option for survival in many patients suffering hepatic dysfunction after liver transplant, but it is associated with significantly increased hospital costs and inferior outcomes compared to primary transplants. In this review, the indications for both early and late liver retransplant are presented. The controversy regarding retransplant in patients infected with hepatitis C virus is discussed, and the recipient and donor graft characteristics impacting survival following hepatic retransplant are analyzed. With improved risk stratification models to match retransplant candidates with appropriate donor grafts, it is hoped that the clinical outcomes of retransplant will continue to improve.
