Introduction Ovarian sarcomas are extremely aggressive gynecological malignancies with extremely unfavourable prognosis especially in cases diagnosed in advanced stages. Even so, it seems that the only way to improve the prognosis of these patients is debulking surgery to no residual disease. Material and method We present the cases of 5 patients diagnosed with advanced ovarian sarcomas submitted to debulking surgery with radical intent. Results and conclusions The main associated resections consisted of total hysterectomy with bilateral salpingo-oophorectomy in 4 cases, left salpingo-oophorectomy in one case (in the case of a patient who had previously suffered a complete hysterectomy with right salpingo-oophorectomy for benign pathology), rectosigmoidian resection in 3 cases, segmental enterectomies in two cases, partial cystectomy en block with distal urethrectomy and ureteral reimplantation in one case, total omentectomy in 5 cases and pelvic and parietal peritonectomy respectively in 3 cases. In all 5 cases a maximal cytoreduction was achieved. Regarding the histopathological examination, it confirmed the presence of ovarian carcinosarcomas in 3 cases while in the other 2 cases the diagnosis was the one of an ovarian leiomyosarcomas. Postoperatively a single patient developed a postoperative hemoperitoneum and required reintervention. In conclusion, patients with ovarian sarcomas can safely undergo debulking surgery in order to provide an adequate local control of the disease.
