We reported our experience in 42 patients with pineal region tumors over 26 years. Twelve patients underwent surgical resection. Histologic diagnosis was made in 13 tumors, while the others were diagnosed according to clinical features and imaging studies, cytology, tumor markers, and response to radiotherapy. Total surgical removal was achieved in 8 patients (66.7%), subtotal removal in 1 (8.3%), and partial removal in 3 (25%). Morbidity was negligible; postoperative complications resolved within 1 to 2 weeks. No mortality occurred. One patients had recurrence, and one had dissemination. Choices of preoperative studies and surgical technique to optimize outcome are discussed.
