Immunoablation with autologous hematopoietic stem cell rescue has been used in over 1,300 autoimmune disease patients, around 150 with SLE. Some patients have experienced durable remissions with loss of autoantibodies, whereas others either did not respond or died as a result of the treatment. Prospective randomised trials are required and are being planned to establish the place for this potentailly curative strategy. Mesenchymal stem cells are in an exploratory phase for the treatment of acute autoimmune disease including SLE. The principle is that they home to inflammed tissue and exert an antiinflammatory paracrine effect. Lupus (2009) 18, 387-393.
