Patients with multifocal and lung-limited minimally invasive adenocarcinoma and limited pulmonary reserve can rarely be candidates for lung transplantation. This treatment modality may offer extended palliation in highly selected patients when systemic therapies often yield survival of only 1 year. Preoperative confirmation of tumor histology and mediastinal staging ensure appropriate patient selection and are essential to ensure appropriate candidacy. Lung transplantation can be safely performed in this small and highly selective patient population, with expected survival rates approaching the general lung transplant population.
