Hypnic headache - a "new type" of headache disorders

Hypnic headache has been described in several case reports since 1981 and is regarded as idiopathic headache disorder of the elderly. In this review, the clinical features, neurophysiological including polysomnographic findings, and treatment procedures are analysed. There is evidence that hypnic headache is a REM-sleep disorder. The analysis shows that hypnic headache most probably is on entity among the idiopathic headache disorders unassociated with structural lesions and does not belong to the trigemino-autonomic headaches. Lithium shows the best efficacy; indomethocin, flunarizine, and caffeine might have prophylactic efficacy as well.
