A case report of Laparoscopic Abdominoperineal resection of rectum for cancer lower rectum is presented. In this cost effective technique suitable for developing countries neither a GIA stapler was used nor any blood transfusion. Four abdominal ports were used, one umbilical, two lumber and one in right iliac fossa. Clips and cautery used for dissection of mesorectum and a penile catheter was used to pull out the sigmoid colon which was divided outside and the distal portion pushed back in tile pelvis. The specimen was taken out through the perineal wound. Patient recovered fast without any complication. Eight months follow-up was complication free.
