Use of Autologous Platelet Gel in Female Pelvic Organ Prolapse Surgery: A Feasibility Study

Study Objective: We sought to evaluate whether autologous platelet gel (APG) application during anterior colporrhaphy could increase collagen content of the pubocervical fascia and thereby create a more durable repair. Design: Prospective cohort study (Canadian Task Force classification II-2). Setting: University hospital. Patients: Nine patients undergoing anterior colporrhaphy. Interventions: We applied APG to the surgical site during anterior colporrhaphy in 9 patients. We collected biopsy specimens front the anterior vaginal wall at surgery and 3 months postoperatively. Collagen content was measured and compared. We evaluated objective and subjective failure rates at 20 months postoperatively. Measurements and Main Results: No significant increase occurred in collagen content at 3 months. Subjective failure rate was 12.5%, objective failure rate was 66.7%, and 1 (12.5%) patient required reoperation 10 months postoperatively. Conclusion: The application of APG during anterior colporrhaphy does not seem to increase collagen content or durability of the surgical repair.
