Cholangiocarcinoma involving the distal common bile duct (distal cholagiocarcinoma [DCC]) is a periampullary neoplasm that is less common than, but often difficult to distinguish from, pancreatic adenocarcinoma (PDA). Although the prognosis and cure rate of DCC is improved over that of PDA, it remains a highly lethal disease. While the pathophysiology of DCC is, in many instances, distinctly different form PDA, the diagnostic and therapeutic management of this disease is not dissimilar from pancreatic cancer. A multi-disciplinary approach toward DCC is important.
