From January 2006 to May 2012, 34 consecutive patients with angiographically confirmed very late stent thrombosis (ST) after bare-metal stent (BMS) implantation were identified at our center. Median period from BMS implantation to ST was 7.5 years and most of the patients were receiving oral antiplatelet therapy at the time of ST. Clinical presentation was STEMI in 31 patients (91%) and all patients were treated with percutaneous coronary intervention (PCI). A significant deterioration of LVEF occurred after very late ST (62+/-10% to 50+/-11%; p<0.001). MACE (cardiovascular death and myocardial infarction) rates were 14%, 26% and 43% at 30 days, 1-year and 2-year follow-up, respectively.
