This editorial discusses what constitutes ''persistent'' advanced block at the atrioventricular node in patients with inferior wall acute myocardial infarction, a class II indication for permanent pacing according to the ACC/AHA guidelines for pacemaker implantation. Review of the literature suggests that permanent pacing is rarely indicated in this situation.
