APPLICATION VALUE OF CARBON NANOPARTICLES IN MULTIFOCAL PAPILLARY THYROID CARCINOMA

摘要
Objective: To investigate the application value of carbon nanoparticles (CN) in the operation for multifocal papillary thyroid carcinoma (MPTC). Methods: A total of 100 patients with MPTC were recruited and divided into CN group (n=50) and control group (n=50) according to whether the patient injected carbon nanoparticles during the operation with informed consent. The postoperative pathological conditions , complications , and follow-up results were compared between the two groups. Results: A total of 588 lymph nodes were dissected in the CN group and 315 lymph nodes in the control, the mean number of dissected lymph nodes was statistically higher in the CN group than in the control(P<0.05). The number of lymph nodes <5 mm in the CN group was 282 , while there was 82 in the control group,the differences was statistically significant (P<0.05). The mean number of metastatic lymph nodes and <5 rum metastatic lymph nodes in the CN group was statistically higher compared to the control(P<0.05). Parathyroid gland was miscut in 1 case in the CN group and 8 cases in the control.The control group had a relatively higher incidence of hypocalcemia and hypoparathyroidism compared to the CN group (P < 0.05).The recovery time of parathyroid function in the CN group was significantly less than that in the control group (P<0.05), serum PTH in the CN group were significantly higher than those in the control group on postoperative day 1,week 1, and month 1(P < 0.05). The serum Ca2+ in the CN group was significantly higher than that in the control group on postoperative day 1(P<0.05). Conclusions: The application of carbon nanoparticles in MPTC surgery can effectively improve the number of lymph node dissection and metastatic lymph node dissection, especially lymph nodes <5 mm, which is beneficial to the complete dissection of central lymph nodes, and significantly reduces postoperative complications, accelerates the recovery of parathyroid function, improves the quality of life of patients, and reduces postoperative recurrence.
