Massive hemorrhage from a pulmonary vein-esophageal fistula: a late complication of Histoacryl glue injection

Injection of N-butyl-2-cyanoacrylate (Histoacryl glue) is superior to variceal band ligation (VBL) and sclerotherapy in achieving hemostasis of bleeding gastric varices.(1-3) Infrequent complications include sepsis, distal embolization of glue, and mucosal ulceration.(4) Histoacryl glue is less frequently used in the esophagus because of the widespread acceptance of band ligation and sclerotherapy and conflicting evidence of comparative efficacy.(5-8) We report the development of an esophageal sinus as an early complication after esophageal injection of Histoacryl glue, with Subsequent massive hemorrhage from an esophageal-pulmonary vein fistula as a late, fatal complication.
