Which dialysis modality is best for patients with diabetic nephropathy?

摘要
The numbers of patients with type 2 diabetes is increasing worldwide. Correspondingly, there are also more patients with diabetic nephropathy. The major goal is a preemptive kidney transplantation; however, if that is not feasible dialysis treatment is indicated. Older German guidelines recommended peritoneal dialysis (PD) as the modality of choice for patients with diabetes. The arguments were better survival in the first years of treatment, better preservation of residual renal function and other factors, such as quality of life and mobility. Nowadays, experts recommend an individualized selection regarding the choice of the modality of dialysis, even for diabetic patients. It is therefore astonishing that the numbers for PD in Germany are low. For patients who have received adequate clarification and have an open choice of procedure, the proportion of PD is around 30-50% and this should be the goal for patients with diabetes in the future.
