Incidence of Clostridium difficile infections has significantly increased in the US, Canada and Europe over the last 10 years, bringing to the forefront the need of a reliable and rapid diagnosis. This diagnosis is based on toxin detection or on the recovery of a toxigenic strain in diarrhoeal stools. Many tests are now available; some can detect toxins, bacteria or their components, and some detect the toxin genes. However, the multiplicity of methods does not facilitate the choice of clinical labs; moreover, none of the methods satisfy all the criteria of an ideal method in terms of sensitivity, specificity, rapidity and cost. Currently, the best strategy is based on a 2- or 3-step approach. (C) 2011 Elsevier Masson SAS. All rights reserved.
