Monopolar electrocautery with spray coagulation in laparoscopic colectomy

For safe laparoscopic colectomy, it is important to avoid haemorrhage. The main aspects of the procedure are dissection and mobilisation. If the operating field is contaminated, even by just a small amount of blood, it becomes difficult to accomplish the procedure safely because the correct layer of dissection is obscured. For dissection, we use monopolar electrocautery with spray coagulation. With this coagulation method, the cauterised tissue does not adhere to the electrode and the depth of coagulation is shallow. The appropriate layer of the spray-coagulated fragile membrane can be bluntly dissected very easily. To maintain the proper layer of dissection and sufficient haemostasis, spray coagulation using monopolar electrocautery is recommended for laparoscopic colectomy.
