Background Fibroepithelial stromal polyps are benign lesions of the vulvovaginal region that usually appear bland and hypocellular and thus easily recognized. However, sometimes they exhibit hypercellularity and bizarre morphology with atypical mitotic figures, often raising the possibility of malignancy. Case We describe a case of a 22-year-old primigravida female who presented with a mass protruding from vagina. Results Hypercellular areas showing atypical cells with moderate to marked pleomorphism and numerous mitotic figures were seen. Conclusion A potential pitfall on biopsy which may lead to classifying these benign lesions as malignant based on atypical morphology.
