Heparin is one of the oldest drugs on the market. Widely used for the prevention and treatment of thromboembolic diseases, its adverse effects are well-known. Allergic reactions are rare (0.2%), especially immediate hypersensitivity reactions, which are exceptional. We report a case of an immediate allergic reaction to nadroparine. We discuss diagnostic methods, the potential interest in the basophile activation test, cross-reactivity and therapeutic alternatives. (C) 2010 Elsevier Masson SAS. All rights reserved.
