Drug-related complications in regional anaesthesia - An analysis of CIRS-AINS

Background: Critical incidents related to regional anaesthesia can be dangerous for patients. Method: We analysed the critical incident reporting system database of the German Society of Anaesthesiology and Intensive Care Medicine and the Professional Association of German Anaesthesiologists. Results: It appeared that the majority [of the incidences] were related to drug use and preventable (89%). Critical incidents in regional anaesthesia when drugs are used are most often associated with local anaesthetics or a wrong line application to the epidural or intrathecal space. Conclusion: Characteristic labelling of lines, syringes and connectors is a possibility for preventing many of the incidents. Training of personnel involved in all relevant procedures and a strict separation of drugs prepared for full anaesthesia and regional anaesthesia can also help lowering the rate of incidents. Critical incident reporting systems (CIRS) provide an important possibility for analysing incident causes. Communicating incidents to CIRS can help improving patient safety.
