Endoscopic retrograde cholangiopancreatography (ERCP) in patients with Roux-en-Y gastric bypass remains a major clinical challenge and has traditionally been performed by 1 of 2 improvised methods: (i) using balloon-assisted enteroscopy or (ii) advancing the ERCP scope through a gastrostomy tract into the excluded stomach and duodenum. Recently, endoscopic ultrasound (EUS)-guided creation of a transluminal fistula into the excluded stomach for ERCP was described and is gaining popularity (1). © 2019 by The American College of Gastroenterology.
