摘要
This paper focuses on how small changes in technological systems may create latent conditions for organisational failures and increased risks. Our main hypothesis is that small changes in one part of a complex technological system will generate higher risks than big changes in the system. Our perspective will be illustrated by a case from the Norwegian railway system more specifically the Aasta accident in 2001. The aim of the paper is to discuss the vulnerability of technological systems when constantly being confronted with external requirements of incremental adjustments and adaptations. However, it is important to underline that this paper does not attempt to present a general theory on latent failures, but to emphasise how goals of efficiency and innovation in certain situations may be contradictory.
