Symptomatic ossification of ligamentum flavum (OLF) in the lumbar spine is a rare disorder. We report on a patient with lumbar OLF and spondylolisthesis grade 1. Our patient was admitted to our hospital with symptoms of neurogenic claudication. Plain radiographs showed spondylolisthesis grade I at the L3-L4 level. Magnetic resonance imaging (MRI) revealed a dorsal hypointense lesion, whereas computed tomography (CT) demonstrated radiodense lines creating a characteristic V-shape at the same level. Postoperatively, the neurological status improved significantly. It is possible, that hypermobility due to spondylolisthesis has contributed to the ossification procedure.
