Primary erythrocytosis in six cats

Primary absolute erythrocytosis is a rare myeloproliferative disease in cats. Unlike secondary erythrocytosis it is characterized by both the lack of a hypoxia-triggered disease and an erythropoietin secreting tumor. Six cases of feline primary erythrocytosis are described retrospectively. Diseases causing secondary erythrocytosis were excluded with the aid of clinical, laboratory and diagnostic imaging procedures. Five Domestic Shorthair and 1 Siberian Forest cat (2-11 years, median 4, 5 neutered male, 1 neutered female) were included. Reasons for presentation were seizures (n=4), vomiting (1) and a geriatric check-up (1). Three cats were hypothermic (rectal temperature 35.6-37.8 degrees C), 2 had red mucous membranes and congested episcleral vessels, and one cat each had fasciculations (1) and tachypnea and hypersalivation (1). The hematocrit ranged from 0.59 to 0.78 I/I (median 0.68). The initial serum erythropoietin concentration of 5 cats was <0.1, 3.0, 6.8, 8.6 and 39.3 U/I (reference range 0-6). The latter two values were checked after 9 and 10 days, respectively (3.8 and <0.1 U/I, resp.). Initial therapy included infusions (5), phlebotomy (6) and phenobarbital or diazepam (4). All cats were treated with hydroxyurea and if necessary further phlebotomies. Two of the cats were euthanized 3.5 months and 4.5 years after initial presentation due to refractory seizures or methemoglobinemia, respectively. Follow-up was not possible in 1 case and only available for 3 months in the other. The 2 remaining cats are 10 and 22 months after the start of therapy still under treatment.
