DIABETES MELLITUS AND METABOLIC SYNDROME IN PRIMARY HYPERPARATHYROIDIISM

Background: Primary hyperparathyroidism (PHPT) was associated with increased prevalence of diabetes mellitus and increased cardiovascular morbidity and mortality. Patients and methods: 64 patients (4M/60F), aged 56.8 +/- 12.2 years were retrospectively assessed. Serum PTH was measured by immunoradiometric assay, 25 OH vitamin D by radioimmunoassay. Results: Mean body mass index was 27.30 +/- 4.9 kg/m(2); 45.3% of patients were overweight, 18.8% were obese. 16 patients (25%) showed. normocalcemic PHPT. Type 2 diabetes mellitus was recorded in 8 patients (12.5%), impaired glucose tolerance in 7 patients (10.9%), dyslipidemia in 34 cases (53.1%), arterial hypertension in 37 patients (57.8%). There was a positive correlation between trigfycerides and PTH (r=0.385, p=0.004), a negative correlation between HDL-cholesterol and PTH levels (r=-0.416, p=0.014). Prevalence of impaired glucose tolerance and type 2 diabetes mellitus was similar in normocalcemic and hypercalcemic PHPT. Conclusion: There is a high prevalence of diabetes mellitus and other cardiovascular risk factors in patients with PHPT.
