Both smoking and alcohol consumption are commonplace and could change with lifestyles. The effects of combined smoking and drinking exposure on testosterone and related indexes in men of different age groups remain to be studied. This study aimed to study the effects of smoking and drinking on testosterone and related indexes in men of different ages. Stratified cluster sampling was used to select adult males over 20 years old from 7 seven communities in Zunyi between August 2013 and September 2013, 968 met the eligibility criteria were enrolled. The study participants were categorized into the smoking and drinking group (SD group), non-smoking and non-drinking group (NSND group), smoking but non-drinking group (SND group), and nonsmoking but drinking group (NSD group). Testosterone and related indexes in venous blood were measured. Chi-square test showed that there were age differences among the four groups (51.53 +/- 15.74 vs 52.69 +/- 12.43 vs 46.17 +/- 12.94 vs 49.62 +/- 12.16, P<0.05). T-test showed that smoking affected CFT (0.35 +/- 0.09 vs 0.33 +/- 0.10, P<0.05) and SHBG (32.90 +/- 15.60 vs 39.34 +/- 17.75, P<0.05) in men under 65, and affects SHBG (49.36 +/- 23.96 vs 60.32 +/- 20.73, P<0.05) in men aged 65 and older. Compared with the NSND group, the SND group had lower CFT(0.32 +/- 0.10 vs 0.36 +/- 0.10,P<0.05) and higher SHBG(41.77 +/- 18.14 vs 34.05 +/- 17.26,P<0.05) and LH(6.04 +/- 3.66 vs 4.88 +/- 2.07,P<0.05) levels in men under 65,the SND group had higher TT(18.29 +/- 5.41 vs 15.55 +/- 5.35,P<0.05) and SHBG(60.60 +/- 20.03 vs 49.16 +/- 26.51, P<0.05) and SD group had higher SHBG(60.12 +/- 21.35 vs 49.16 +/- 26.51, P<0.05) in men aged 65 and above. Multiple linear regression showed that the SND group had higher SHBG than those in NSND group (45.09 +/- 19.80 vs 38.67 +/- 21.51, ss=5.60, P=0.02). SND is more likely to affect CFT and SHBG levels in men under 65, while SHBG levels are more likely to be affected in men aged 65 and older
