Over the last decades lung transplantation has emerged as a considerable therapeutic option for patients with cystic fibrosis (CF) and severe pulmonary disease. in addition to procedure-related difficulties such as shortage of donor organs and chronic allograft rejection disease, there are numerous specific issues related to CF and severe pulmonary disease that need to be considered. It is the aim of this article to summarize some important aspects to be considered when advising CF patients who are potential lung transplant candidates. Attention is focused in particular on: appropriate time of referral, post-transplant outcome, and issues of possible future challenges in this population.
