Endoscopy plays an essential role in diagnosis, management, and surveillance of inflammatory bowel disease (IBD). Direct visual and histologic evaluation allows the distribution, severity, and disease activity to be defined. In patients with established IBD, the response to both medical and surgical therapy can be assessed. Furthermore, endoscopy has a therapeutic role in IBD in management of strictures and hemorrhage. In patients with longstanding disease, endoscopy plays an important role in dysplasia surveillance. This review aims to update the role of endoscopy in IBD, with particular reference to the initial diagnosis, disease evaluation and monitoring, dysplasia surveillance, and therapeutic functions.
