The majority of patients with Crohn disease will undergo at least one surgical resection during their disease course. Currently, the most effective prognostic indicator of postoperative disease course is the Rutgeerts score, determined during ileocolonoscopy 6 to 12 months following surgery. Patients with endoscopic evidence of recurrence have a significant risk of symptomatic recurrence, and complications. Prophylactic therapy should be considered in patients who have risk factors for postoperative recurrence, and treatment should be escalated if there is evidence of recurrence at postoperative ileocolonoscopy. An algorithm for the evaluation and treatment of postoperative Crohn disease is presented.
