Intercostal Video-Assisted Mediastinal Surgery through an intercostal window (IVAMS): A simpler approach to perform mediastinal parathyroidectomy

ALTHOUGH ECTOPIC PARATHYROID ADENOMAS have been reported to occur in 4 to 25% of patients with hyperparathyroidism, (1-4) most of these adenomas can be excised through standard cervical approach. Thoracic parathyroid ectopia is an uncommon situation causing persistent or recurrent hyperparathvroidism.(5) Minimally invasive techniques have been developed as an alternative to sternotomy or lateral thoracotomy.(6-8) We present a new approach to perform ectopic parathyroidectomy consisting of an Intercostal Video-Assisted Mediastinal Surgery through a unique intercostal window (IVAMS); it was developed as an alternative to standard Video-Assisted Thoracic Surgery (VATS) in order to simplify, if possible, the management of these infrequent operative cases.
